To Initiate your personal monthly giving plan, simply complete the following steps:

  • Step One: Print out this page and fill in the amount of your contribution above.
  • Sign and date the authorization form below.
  • Step Two: Include a void cheque from your desired banking account.
  • Step Three: Return your completed authorization form to RIH Foundation,
    311 Columbia Street, Kamloops BC V2C 2T1.

I authorize Royal Inland Hospital Foundation to deduct the specified amount from my Bank Account on the 15th of each month until I notify them in writing to discontinue this payment.
